Building a HIPAA-Compliant Practice Automation SaaS with Multi-Day Patient Sequences and Real-Time Voice AI.
The Challenge
Healthcare clinics spend substantial administrative hours running manual patient follow-ups, schedule audits, and billing reconciliations. ROVA Health needed a multi-tenant patient engagement and practice automation platform to coordinate clinic communications (SMS, WhatsApp, emails, and voice calls) across thousands of patients. The core engineering challenge lay in executing complex, multi-day patient outreach sequences without losing state during server restarts, while maintaining strict HIPAA compliance and zero-leak data paths.
The Solution
ClarvoTech built a multi-tenant platform architected as nine polyglot microservices behind a React 18 dashboard. We deployed Temporal.io to manage long-running patient outreach workflows, ensuring fault-tolerant execution of communication steps. The core backend utilizes ASP.NET Core (.NET 8) with MediatR CQRS over EF Core 9 and PostgreSQL. We implemented two Python FastAPI services for real-time operations: a voice AI assistant powered by LiveKit to handle calls, and a WebSocket-based conversation manager. The system secures sensitive records using AWS Cognito RS256 token verification, HTTPS encryption, and granular HIPAA-compliant audit trails.
The Process & Implementation
Our database architects mapped clinic data schemas, creating isolated tenant structures to ensure HIPAA privacy bounds. We integrated Twilio and Stripe to support multi-channel notification drops and direct insurance/co-pay transaction payments. We built a C# on-premises sync daemon that securely uploads clinic data to AWS, writing real-time logs to CloudWatch. We executed continuous integration tests to verify database performance under massive read-write loads before scaling production services.
